Boston, MA 06/12/2014 (wallstreetpr) – Leading chain of drug store operator, Rite Aid Corporation (NYSE:RAD) disclosed that Penn State Hershey Health System has joined its program of Rite Aid Health Alliance. The partnership will allow them to provider community-based, personalized health management coach to poly-chronic and chronic patients in Greater Harrisburg and Hershey.
Rite Aid Health Program
The drug store operator had launched an initiative earlier this year with a view to offer comprehensive healthcare and support to achieve health improvement objectives established by their physicians.
Rite Aid health program offers a wide range of services to patients including comprehensive medication reviews, compliance support, reconciliation, weight and nutrition management information, educating on disease, coaching of exercise and cessation support for tobacco users. Rite Aid Corporation (NYSE:RAD) plans to store all interactions electronically during the program, and also any updates provided by the physicians including profiles of patients, lab results, and medication. The program helps continuity of care and offers convenience information access to other members of the patient’s team of healthcare service provider.
The program is an integrated care model and is the first of its kind in the drug store industry since it leverages the combination of community pharmacists’ expertise and the specially trained healthcare coaches from in-store.
Chronic Conditions
Through Rite Aid Health Alliance initiatives, patients with poly-chronic and chronic conditions, like diabetes, high cholesterol, congestive heart failure, and COPD are recommended by their physicians to the program. The company’s pharmacists and coaches from Rite Aid pharmacies’ different locations work with the physicians and patient to improve the patient’s self-management abilities and health generally.
The health objectives are established by its care team members through collaboration with patients to eliminate any barriers and produce the action plan of a personalized health care by coordinating with the physicians of the patient.
Existing Partnerships
Presently, Rite Aid is aligned with High Point, Cornerstone Health Care, Apollo Medical Holdings, Inc. and Greater Buffalo United Accountable Healthcare Network. Rite Aid Corporation (NYSE:RAD) provides the health initiative program in its selected pharmacies in some of the selected locations.
