Multi-billion-dollar government benefit fraud cases in Minnesota are making headlines for being so pervasive and brazen. But the Minnesota fraud machine<\/a> is not an isolated concern. Benefit fraud is a big business across\u00a0dozens\u00a0of states and\u00a0scores\u00a0of benefit programs.\u00a0I say this because I saw it firsthand as the former head of all FBI criminal investigations and later Global Security Director at on of the largest financial institutions in the U.S., where we used advanced data analytics to detect fraud schemes.<\/p>\n

This is not a new problem.\u00a0An April 2025 GAO report estimated that since 2003, taxpayer losses from fraud in state-administered benefit programs such as SNAP and Medicaid exceed $2.3 trillion. Based on the same GAO data, losses from fiscal years 2018 through 2022 alone range from $233 billion to $521 billion.\u00a0The exposure of the Minnesota fraud machine\u2014and the remediation that follows\u2014may ultimately be the greatest accomplishment of Tim Walz\u2019s political career.\u00a0A 2025 California State Auditor\u2019s report put that state\u2019s benefit fraud at more than $70 billion.<\/p>\n

Among the largest programs that have been exploited are Medicare and Medicaid. For too long, unscrupulous\u00a0health care\u00a0providers have\u00a0used\u00a0strategies such as medical identity theft, billing for unnecessary services or items, billing for services not furnished, upcoding, unbundling, and kickbacks to\u00a0unfairly profit\u00a0from Medicaid programs<\/a>. According to the National Health Care Anti-Fraud Association, about\u00a03%\u2014roughly $300 billion\u2014\u00a0of health care spending is lost to fraud annually. That\u2019s a bonfire fueled by taxpayer dollars.<\/p>\n

CONGRESS OPENS \u2018INDUSTRIAL-SCALE FRAUD\u2019 PROBE IN MINNESOTA, WARNS WALZ DEMANDS ARE \u2018JUST THE BEGINNING\u2019<\/strong><\/a><\/p>\n

The Minnesota scandal has at long last brought this seedy situation into full public view and, ironically, may become the catalyst for initiatives that finally\u00a0staunch the long-standing hemorrhage of taxpayer funds. A sleeping giant has been awakened: taxpayer outrage and an administration now under pressure\u00a0to act. Minnesota will be ground zero to unleash the FBI, IRS, and various\u00a0inspectors general, who have\u00a0long been limited\u00a0to chasing whistleblower complaints one by one in a wasteful game of whack-a-mole. Reliance on sporadic tipsters is not an effective way to confront systemic fraud.<\/p>\n

Make no mistake, significant indictments will follow for years to come because of the Minnesota benefits crime tsunami. Arresting and prosecuting criminals\u00a0works\u2014it deters future crime.<\/p>\n

TREASURY SECRETARY ANNOUNCES CASH REWARDS FOR MINNESOTA FRAUD WHISTLEBLOWERS<\/strong><\/a><\/p>\n

But law enforcement<\/a> is the tail end of the process.\u00a0The real failure comes earlier:\u00a0programs must be administered responsibly, and crimes must be detected before the money is gone. The ancient Greek tragedian Sophocles famously said, “What is not sought will go undetected.” That maxim applies perfectly to fraud schemes draining\u00a0taxpayer\u00a0funds.<\/p>\n

What federal prosecutors have described as “industrial-level” fraud\u00a0was predictable and detectable. States willing\u00a0to\u00a0use modern tools\u2014especially artificial intelligence<\/a>\u2014will find it. Simple steps, such as allowing the\u00a0U.S.\u00a0Treasury Department\u2019s “Do Not Pay” system to\u00a0cross-check\u00a0beneficiaries against\u00a0Social Security death records, were implemented only recently. Meanwhile, financial institutions and insurance carriers have\u00a0used data analytics\u2014now marketed as AI\u2014\u00a0for decades to prevent fraud. Banks deploy\u00a0predictive analytics, anomaly detection, and network analysis\u00a0because they have a financial incentive to do so.<\/p>\n

Although CMS and other stakeholders have attempted to install safeguards, evidence suggests Medicaid programs are still losing billions. Traditional “pay and chase” approaches remain ineffective. The system\u00a0shovels benefits out the door, then attempts recovery long after the damage is done. But there is something even more troubling than sheer incompetence.<\/p>\n

The uncomfortable truth is that blue enclaves like Minnesota<\/a>, California and New York\u00a0often lack the political will to aggressively detect fraud because permissive systems serve electoral goals. Showering large blocs of friendly voters with taxpayer funds\u00a0wins elections. That same logic fueled\u00a0open-border policies under the Obama and Biden administrations. Ungoverned benefits programs and illegal migration translate into votes and campaign cash for the facilitators.<\/p>\n

Case in point: under Gov. Tim Walz, Minnesota has experienced the largest increase in SNAP benefits\u00a0in the nation. Between 2018 and 2023, benefits surged\u00a0128%, followed by Massachusetts, Vermont and California at\u00a0120%, 96%, and 89%, respectively. It is no accident that\u00a082 of the 92 defendants\u00a0indicted in Minnesota child nutrition, housing services and autism program fraud schemes are Somali. This voting bloc can swing elections. Meanwhile, funds intended for those who\u00a0rightfully\u00a0need help are siphoned off by criminal enterprises.<\/p>\n

There are success stories. A new artificial intelligence tool helped the\u00a0U.K.\u00a0government recover nearly\u00a0\u00a3500 million\u00a0in the past year, according to the BBC, by cross-referencing data across agencies and\u00a0flagging vulnerabilities before they are exploited. Officials say it can help make policies\u00a0effectively fraud-proof\u00a0before rollout.<\/p>\n

States willing to look for fraud will find it.\u00a0The hope is that the Trump administration\u2019s response<\/a> to Minnesota\u2019s systemic malfeasance becomes a template nationwide\u2014red, blue, and purple.\u00a0<\/p>\n

Taxpayers deserve the same relief achieved through firm border enforcement:\u00a0near-zero losses without changing a single law. The goal should be\u00a0zero tolerance\u00a0for sloppy administration and criminal schemes\u2014preventing losses before they occur, not chasing them after the fact.<\/p>\n
